Keep a medicine list you can trust

Keep original prescriptions and a current list ready for a pharmacist or clinician, including questions about anything unclear.

Make the information easy to review

A prescription document, an old list and what you currently take may not all say the same thing. Keep the original source and make it clear when an entry was updated or when something is uncertain.

When preparing for a review, include the medicines and other products a healthcare professional asks about. Avoid relying on memory or assuming that an app can infer a current regimen from an old document.

Keep treatment decisions with qualified care

If an instruction is unclear or you think there may be a problem, speak with a pharmacist or clinician. Do not start, stop or change a medicine or supplement based only on an AI explanation or a decorative reminder.

A medicine list can help organize the conversation, but it is not a medication reconciliation service unless the relevant clinical process and responsibilities have actually been established.

A connected handover still needs checks

Afiyah can help keep prescription information and a prepared request connected to the person’s records. A partner pharmacy must verify the prescription, availability, price and dispensing requirements. Delivery needs a confirmed service and details.

The website explains the intended sequence. It does not place an order or promise immediate delivery. The helpful experience is a clear next step with honest confirmation, rather than a status that only looks finished.

A small checklist to keep

  • Keep the medicine names and instructions accurate.
  • Include nonprescription products and supplements in a professional review.
  • Ask a pharmacist or clinician about uncertainty; do not guess.
