Make your next care visit count

The right records, an up to date medicine list and the questions you want answered can make preparation clearer.

Choose relevance over volume

Having every file on your phone is different from knowing which one you want to discuss. Before a visit, identify the report, note or question that prompted the conversation. Keep the correct person, source and date attached.

You can also ask the service what information it needs beforehand. Digital and in person visits may have different arrangements. Do not assume that saving a request in an app means a clinician has received it or that an appointment is confirmed.

Make the questions easy to find

A short list can help you remember what matters to you. You might ask what a term means, whether a result needs follow up, how to follow an instruction, or who to contact if the agreed next step is unclear.

At the end, check that you understand what was agreed, what remains uncertain and what happens next. If something does not make sense, asking for another explanation is a reasonable part of care.

Carry the story forward, with permission

Afiyah’s current internal app prepares and tracks care requests. The connected care vision adds verified provider handovers, selected records and confirmed updates, with your approval. Provider availability, fees and booking still require confirmation.

Keep the clinician’s original notes after the visit and distinguish them from a generated summary. Insurance paperwork, prescription requests and follow up are separate handovers with their own responsibilities; a smooth design does not remove the need for those checks.

A small checklist to keep

  • Choose the relevant original records.
  • Bring an accurate medicine and supplement list.
  • Write down the questions and next step details you need.
